Instructions for submitting an application for funding:
Complete the Grant Application Form.

Submit the original application along with four copies of all materials.

Attach a list of the organization’s governing board.

Attach an overall operating budget for the organization.

Attach a copy of your IRS tax-exempt letter. The Foundation will normally only make grants to organizations that are tax-exempt under Section 501(c)(3) of the Internal Revenue Code. Under some circumstances, the Foundation may make grants to other tax-exempt organizations provided the funds are used for charitable purposes.

Applications must be postmarked by Friday, April 13, 2007.

Grant Proposal Description

Please attach the necessary pages to the application’s cover page in answer to the following questions. Responses must be numbered, typed in a font no smaller than 10 points and not in excess of three pages.

1. ORGANIZATION: What is the purpose of your organization and whom does it serve?

2. NEED: Why is there a need for this project or program in the community? How was the need determined?

3. WHO: Who will be served by this project? How many will be served?

4. PROGRAM OBJECTIVES: What will this project specifically accomplish? (Please list measurable program objectives.)

5. PERSONNEL: Describe the organization’s capacity to undertake the proposed project. Describe any volunteer activities anticipated for this project.

6. METHOD: How do you plan to implement your idea? Describe the specific sequence of activities necessary to accomplish the program objectives and how Foundation dollars will be spent.

7. PUBLICITY: How will you publicize the activities and/or the results of the project?

8. FUNDING: What is your projected budget for the project, including grant expenditures? What other sources of funding are there for the project? If this is an on-going project, how will it be financed in the future?

9. EVALUATION: How do you plan to measure progress and determine the ultimate success of the project?
